Join the HTS Street Team or other available positions as we Hit the Streets of Cleveland, East Cleveland & Surrounding areas in Cuyahoga County providing meals, critical resources and enrolling individuals into one of our long-term support programs for further assistance.
Whether we’re enrolling youth into our Gamechangers Youth Initiative or Adults with Medicaid waivers into our Community Integration program or assisting adults with career, housing and financial goals, we’re here to help make a lasting impact in the community and we’re looking for dedicated volunteers to help us guide and support individuals on their journey to long-term stability.
Who We Need:
Street Team members/ Mobile Outreach
Support our communities by getting in the fight against food insecurity and poverty with our Mobile Outreach Street Team.
Youth Outreach Leaders
Help us Support, Empower and Lead our young leaders to success through our Gamechangers youth initiative where we host a variety of youth outreach efforts, mentoring and community events.

Community integration Program (CIP)
Supporting individuals through Medicaid funded community integration and long-term stability.
Our Long-Term Support initiative delivers structured, Medicaid-funded community integration services designed to support individuals with daily living skills, community connection, and sustained independence. Services are provided through approved Medicaid waivers and coordinated in alignment with individual support plans. Learn More Here
